To complete the repair process, simply complete our “Repair Order Form”. After completing the “Repair Order Form”, Click “Submit". Print the email you receice from CSI and send it with the Equipment. We recommend that you do not send any accessories unless requested by the Repair Dept. For shipping instruction Click Here
